Veterans

Please attach to your application a photocopy of your DD214, DD215, NGB-22, Statement of Service, or other appropriate military documents, including, but not limited to, a copy of your initial military contract to verify the length of your initial service commitment.

Surviving Spouses of a Deceased Veteran

Please attach to your application a photocopy of your deceased veteran-spouse’s DD214, DD215, NGB-22, Statement of Service, or other appropriate military documents, including, but not limited to, a copy of your deceased veteran-spouse’s initial military contract to verify the length of their initial service commitment.  Additionally, please attach proof of marriage to your deceased veteran-spouse, a certified copy of your decease veteran-spouse’s death certificate, and a statement that you have not remarried after the death of your veteran-spouse.

Spouses of a Disabled Veteran 

Please attach to your application a photocopy of your veteran-spouse’s DD214, DD215, NGB-22, Statement of Service, or other appropriate military documents, including, but not limited to, a copy of your veteran-spouse’s initial military contract to verify the length of their initial service commitment.  Additionally, please attach proof of marriage to your veteran-spouse, a copy of a letter from the Federal Department of Veterans Affairs verifying your veteran-spouse’s disability as permanent and total, and a signed statement from your veteran-spouse acknowledging the use of veterans’ preference based on their military service.
